Chronic diarrhea or IBS-D: May have laxative effects via anthraquinone-like activity which may worsen stool frequency, urgency, or looseness.
Electrolyte imbalance / dehydration risk: Because of its mild laxative action, overuse may contribute to low potassium (hypokalemia risk in sensitive individuals) and dehydration in frail or elderly patients.
Diabetes or blood sugar management: Traditional versions often contain sugar (sucrose/mishri base). Use caution in cases of diabetes due to glycemic load (depending on dose/formulation) and interference with glucose control in diabetes.
Heart disease or antiarrhythmic medication: Possible potassium loss may worsen arrhythmia risk, especially with cardiac medicines like Digoxin.
Kidney disease and diuretic medications: Because of its mild laxative effect and possible fluid/electrolyte shifts, use may worsen dehydration and increase the risk of electrolyte imbalance in patients with impaired renal function; caution is advised especially in chronic kidney disease or when taking diuretics.
Bowel obstruction / severe unexplained abdominal pain / GI bleeding: Laxative use should be avoided because it may worsen obstruction or delay diagnosis of serious GI pathology.
Concurrent stimulant laxatives or chronic laxative use: Combined laxative effects may significantly increase the risk of diarrhea and potassium depletion.
Pregnancy: Caution due to mild laxative quality which may stimulate uterine contractions.
